fix: correct HVAC_Event_Form_Builder constructor call with required parameters

Fixes fatal error: 'Too few arguments to function HVAC_Event_Form_Builder::__construct()'
- Added required nonce_action parameter: 'hvac_bulk_event_form'
- Added template_mode parameter: true for bulk operations
